To make use of our application, you are requested to provide your name, email address and mobile phone number. We may, at our discretion, request that you verify the provided information by sending one or more codes to the email or mobile phone number. This verification ensures that the information that is linked to the user account is owned and/or controlled by you.
You will also be asked to select a username and a password.
We securely store this information so that you can log in to the web site to make use of the services offered.
Presently, Eunic-ePHR is offered as a free service and as such we do not collect any financial information. For paid services, we will collect sufficient financial information to process a payment transaction and, if you elect to have recurring automated charges, we will securely retain it for future charges. You may also elect to make individual, one-time payments as needed; in which case we do not store your financial information.
At the end of each session when you log out of the web site you will be presented with a feedback screen. You may, at your option, provide feedback on our service. All such feedback becomes the property of E & EU Akosa Diagnostic Ctr. If you provide feature enhancement suggestions, we may at our option use your idea or a variation of your idea without providing any acknowledgement or compensation to you. At our option, we may from time to time provide a credit to be used towards the purchase of our paid services if your suggestion was adapted; however, we are not required to do so.
If you contact us directly, we may receive additional information about you such as your name, email address, phone number, the contents of the message and/or attachments you may send us, and any other information you may choose to provide.

Log Files
Eunic-ePHR follows a standard procedure of using internet log files. These files log certain information when visiting our website.
